All managers, assistant coaches, and team moms must complete a background check form before participating in practices, games, or other activities associated with their team. Below are the (2) necessary forms for submitting a background check. You will need to download both forms, complete, and fax your forms to 410.486.0731. This is the fax number for EBI, the security company performing the background checks. See www.ebiinc.com for more information.
